FIRST VISIT
Your First Visit is especially important to us.
We are glad you are taking a positive step towards excellent dental health. We are looking forward to meeting you. For more than 20 years, our office has served those who seek gentle quality care in a comfortable environment. We pride ourselves for keeping on schedule and have allowed sufficient time to provide you with our undivided attention.
Prior to your first appointment, you can print a copy of our FORMS so you can comfortably complete them at home.
Please include all medications and important health information so that we can complete a comprehensive exam for you.
Do you need a pre-medication antibiotic?
Please let us know before your appointment. We can also provide you with a letter to request your records be transferred to our office.
When you arrive at our office, we will collect your information and scan copies of your Picture ID and insurance cards into your secure file. We will also ask you to review and sign for our HIPAA documents, which outline our privacy practices to protect your personal information.
We would love for your visit to include a dental cleaning, so our first look has a clear view. Sometimes this may not be possible, for instance, if you are in pain, or need special care immediately. Dr. Kurtzman and his dental assistant may take X-rays and review your history with you to best determine how they can assist you.
Are you interested in upgrading your smile, and maintaining great oral health? We want you to be comfortable with your treatment, so please have your questions ready. We want to know what your dental goals are so we can best help you achieve them!
In most cases we may be able to review your options and proposed treatment plans with you at your first visit. In some instances, we may need more time to review your information and ask you to return to go over your options in detail.
Perhaps you'll want financing or insurance information before scheduling for treatment. We'll be happy to process your insurance pre-determination, and review financing options with you. We are here to assist you and aim to give you a dental experience to smile about.
